I work as a CSW (community support worker)and at times in certain situations I've had to think critically when it came to a med error. Whenever a med error occurs an incident report needs to filled out, pharmacy needs to be called and the individual supported has to be watched carefully for signs that they may be ill.For all these to take place successfully you need to know what exactly occured and think of the possible interactions certain meds have with others, you also need to know what questions you have to ask the pharmacist and how to properly and professionally fill in an incident report.
